Senna for Constipation: How It Works and How Long to Use It

Senna treats constipation by stimulating the colon to contract, and it usually produces a bowel movement 6 to 12 hours after you take it. It is approved for over the counter use and it works. It is intended for occasional, short term relief, not for daily use beyond about two weeks.

Senna for constipation is one of the oldest remedies still in routine medical use, and unlike most of that category it has a clear, well documented mechanism.

How it works

The active compounds in senna leaf and pod are not absorbed in the stomach or small intestine. They travel through intact and arrive in the colon.

Bacteria in the colon break them down. The breakdown products do two things: they mildly irritate the colon lining, which triggers the muscle to contract, and they reduce how much water the colon reabsorbs, so the stool stays softer. Contraction plus softer stool produces a bowel movement.

This puts senna in the stimulant laxative group, which is a different mechanism from a bulking agent like psyllium or an osmotic like lactulose. Stimulants act faster and more forcefully. That is both their advantage and the reason they are not for indefinite use.

Timing and dose

Because it takes 6 to 12 hours, senna is normally taken at bedtime so it works by morning. That is the whole logic of the timing.

Traditional dosing of the dried leaf or pod sits around 0.6 to 2 grams a day. Standardised products state a dose on the pack, and that figure is the one to follow, because the strength of plant material varies far more than a manufactured tablet does.

Start at the lowest dose that works. Taking more because nothing has happened after two hours is the single most common error, and it turns a mild result into cramping and diarrhoea some hours later.

ImportantIf constipation lasts more than a week, keeps coming back, or comes with bleeding, severe pain, vomiting or unexplained weight loss, see a doctor rather than reaching for a stronger laxative. Those can indicate something a laxative will not fix.

Who should not use it

Senna is not suitable for everyone, and some of these are serious.

  • Bowel obstruction. Stimulating contraction against a blockage is dangerous.
  • Appendicitis, or unexplained abdominal pain. Same reason.
  • Crohn’s disease or ulcerative colitis. It irritates already inflamed tissue.
  • Pregnancy and breastfeeding. Ask a doctor first.
  • Children. Only on medical advice.
  • Dehydration or an electrolyte problem. Senna makes both worse.

It also interacts with several medicines. Fluid and potassium loss matters most for anyone on diuretics, on digoxin, or on drugs affected by potassium levels. Anyone on regular medication should check before starting.

The two week limit, and why it exists

Every credible guideline says the same thing: do not use senna continuously beyond about two weeks without medical supervision.

Extended use of a stimulant laxative can leave the bowel less responsive on its own, so a person needs it to have a normal movement. Recovering from that is slower and harder than the original problem. There is also cumulative risk from ongoing fluid and potassium loss.

Short courses, used occasionally, do not carry that problem. The risk comes from continuous use, and particularly from people who started for a reason that has since gone and simply kept going.

For someone who needs day to day regularity rather than occasional relief, a bulk forming fibre such as psyllium husk is the better long term answer, and it does not carry the same use limit. Senna is the tool for a specific, short lived problem.
